The Department of Disabilities and Aging (DDA) is the state agency responsible for providing services and supports to Tennesseans with Intellectual Disabilities.
DDA provides services directly or through contracts with community providers in a variety of settings. These settings range from institutional care to individual supported living in the community.
Regional offices are located in West, Middle, and East TN in Arlington, Nashville, and
Knoxville.
PURPOSE: To define a process for addressing disputes that individuals, families, or BGC Inc. staff regarding decisions made by the agency affecting their participation, rights, or freedom of choices in the services of the
agency.
POLICY STATEMENT: BGC Inc. provides an internal process for resolving issues between individuals and their support coordinators.
To support the individual by gathering resources through the State of Tennessee Regional Office by providing accurate and up to date information regarding all aspects of the person’s life and the changes of the Medicaid Waiver Program as they arise.
In the HCBS Waiver, you must have a Person-Centered Support Plan (PCSP). This is
your plan that helps guide the services and supports you will receive. Your PCSP tells the people who will support you:
• What is important to you-the things that really matter to you.
• What is important for you-the supports you need to stay healthy and safe, and
achieve your goals
Title VI of the Civil Rights Act of 1964 requires that federally assisted programs be free of discrimination. The Tennessee Department of Disability & Aging also requires that its activities be conducted without regard to race, color, or national origin.
As it relates to persons supported in Residential Habilitation and Supported Living levels of need 2-4, the provider may be reimbursed for periods of unsupervised time by requesting the Flexible
Residential Support Rate on behalf of the person.
Consumer Direction (Statewide Waiver (SW) and Comprehensive Aggregate Cap (CAC) Waiver) and Self-Direction (Self-Determination (SD) Waiver) are models of service delivery for home and community-based services (HCBS). They are not services.

“Enabling Technology” is defined as equipment and/or methodologies that, alone or in combination with associated technologies, provide the means to support individuals’ increased independence in their homes, communities, and/or workplaces.

If you qualify for care in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), you have the right to choose between care in your home or placement in an ICF/IID. The Medicaid Home and Community Based Services (HCBS) Waiver program provides Waiver services in your home and community.
SSDI is also known as Supplemental Security Disability Insurance. You receive a payment monthly with SSDI.
SSI is also known as Supplemental Security Income. You receive a check monthly from the government when you have SSI.

Medicaid’s rules, benefits and name can all vary by state. In Tennessee, Medicaid is called TennCare.
Tennessee residents have to meet an asset limit and an income limit in order to be financially eligible for Aged, Blind, and Disabled (ABD) Medicaid through TennCare (Tennessee Medicaid).

These instructions are written as if the head of household is filling out the Renewal Packet. If you are filling the Renewal Packet out for the head of household, you should fill it out from that person’s point of view.
